NEW! Neubert-Webb Trust Grant Applications

NEW! Provider Availability Form - Current providers: If you are open to taking on new individuals, you should complete this form and return it to clsproviderinfo@clarkdd.org by the 5th of each month. You will need to save the form to your computer and then send it to the email address as an attachment. Please note this email is for sending your availability forms ONLY.

NEW! DODD has established a new Support Center to serve you better.  By calling the Support Center, you will be able to speak with DODD staff, instead of leaving a voice mail message.  The phone number is 1-800-617-6733.  The Support Center is open from Monday through Friday from 8 a.m. to 4 p.m.  It is closed on state holidays.  You may leave a voice mail if you call after hours. 

When you call the Support Center, you will be given a list of five options.  Based on the option you select, a knowledgeable staff person will assist you.  The following options are available:

Option 1:  Reporting Neglect or Abuse (These calls are routed directly to the MUI Dept.)
Option 2:  Claims Services and Billing
Option 3:  Provider Certification
Option 4:  Security (System Access, passwords resets, etc.)
Option 5:  All other calls (PASRR, prior authorization, legal, Medicaid Policy, director’s office, etc.)

MUI Reporting Form

Direct Billing Agents - This is a list of agents who meet the technical specifications to direct entry bill claims on behalf of Medicaid service providers. Many of the agents also have billing software that providers may purchase. The list is not exhaustive; there may be other agents that meet the technical specifications required or have billing software available. Inclusion on the list or this site does not constitute endorsement.

Ohio Provider Resource Association (OPRA) Service Directory - The directory provides an extensive database that puts people in touch with providers that serve individuals with disabilities.
