Triple I: Individual Choice, Individual Budget, Individual Plan

  • What is an Individual Budget?  It is the amount of county board funding that is available to an individual.  The Individual Budget has two components: adult  services and transportation.
  • Is this new? No, the Individual Budget has been identified for several years. What is new is that we are informing you about the costs of delivering services and offering you the opportunity to choose the providers.   
  • How is the Individual Budget calculated? The a dult services component is based on assessed individual need and attendance. The transportation component is based on distance from the individual’s home to the service and the individual’s service need.
  • Does everyone get the same amount of funding? No. The funding is dependent on assessed need.
  • What does the Individual Budget for transportation pay for? The transportation budget only pays for transportation to and from adult day services, For example:
    • an agency or independent transportation provider provides your transportation, the agency receives reimbursement per one-way trip based on how far they have to travel to pick you up and/or drop you off.
    • if you are transported by an immediate family member, that provider will be reimbursed at county board mileage rate, or a minimum $3.00 per 1 way trip.
    • you choose a bus pass or
    • any combination of the above for adult  services within your annual individual budget transportation.
  • Do I have to make any changes in my current plan? No, you do not have to change anything in your current plan.
  • Do I have to choose a different provider? No.  However, Individual Budget does provide you the opportunity to make a change if you would like to do so.
  • Can you give me an example of how someone might use an Individual Budget?  An individual is enrolled in an Adult Service program. With an Individual Budget you could:

    (1)  choose to attend a different adult services program OR
    (2) stay right where you are OR
    (3) A combination of the above.

    The total cost of the services cannot exceed the annual Individual Budget and the change must have prior authorization by the county board.  

  • Can I choose providers at any time? Yes. You might decide half way through the year you want to do something different.
  • What do I do? Call your Path Coordinator to discuss your options. Your Path Coordinator will have to check to see if the provider is certified and that funding is available.
